In this role, the successful candidate Responsible for examination, assessment, care planning, and performing physical therapy treatment. Provides treatment intervention that restores bodily functions and prevents disability following injury or disease. The Physical Therapist delegates and monitors responsibilities assigned to support staff and students. Participates in performance improvement activities, and in all mandatory staff education programs. Ensures charge generation as prescribed by business practice.Job Responsibilities:Accurately completes all department and patient care documentation according to current standards and policyAssesses and evaluates the patients rehabilitation needs utilizing observation and interviewing skills. Conducts physical therapy examinations. Utilizes related knowledge and skills on an on-going basis.Develops and implements a care plan that meets the identified needs of the patient and care partners. The care plan must include measurable short and long term goals with time frames.Evaluates the patient response to treatment interventions and revises the care plan with input from appropriate team members, the patient and care partners.Utilizes knowledge of scientific principles and departmental policy to render safe and effective treatment interventions.Evaluates patients progress and response to therapy, and modifies treatment intervention as needed.Provides appropriate educational instruction and clinical supervision for patient, care partners and support staffDemonstrates the ability to organize and manage patient caseload.Develops and implements discharge plan in conjunction with patient, care partners and other members of the care team.Participates in team meetings, case conferences and committees as assigned.Minimum Qualifications:To qualify you must have a Graduate of a professional degree program in Physical TherapyExperience as a Physical Therapist in a hospital or health care setting is preferred.Staff Level I: 0-1 years experience.New York State License or eligible in Physical Therapy CPR certification. Required Licenses: Phys Thrpst License-NYSQualified candidates must be able to effectively communicate with all levels of the organization.
