Job Description

General Summary: The Intake team is designed to smoothly introduce newly enrolled Managed Care patients to Gonzaba Medical Group by collecting a thorough history, performing baseline testing, and communicating GMG services to our patients. Maintains and upholds established patient care, operational expectations, and employee policies in alignment with the clinic operational leadership teams. Supports the Gonzaba coordinated care model by working with all GMG providers and supporting departments/functions to ensure a patient-centered approach is provided at all times.

Supervisory Responsibilities: This position has no supervisory responsibilities.

General Requirements: All duties performed will be done accurately and in a timely manner.

  • Assumes responsibility for maintaining clinical competencies according to Gonzaba Medical Group policy.
  • Ensures customer service is always maintained at the highest level.
  • Exercise tact and courtesy when dealing with patients, visitors, providers, and co-workers.
  • Must always adhere to customer service expectations including in-person and virtual (via telephone, or telehealth applications) communication.
  • Participates in educational programs as needed.
  • Other duties as assigned.

Essential Job Responsibilities

  • Orients all newly enrolled Managed Care patients to Gonzaba Medical Group.
  • Establish a rapport with the newly enrolled Managed Care patients so they immediately keep welcome and confident they have made the right choice selecting Gonzaba Medical Group for their medical needs.
  • Orient the patient to all the in-house services GMG offers including our various urgent care locations, PT/PMR, Nursing Services, in-house Specialists, Radiology, and Lab services.
  • Ensure an appointment with the PCP is scheduled in an appropriate time frame and communicates to the patient.
  • Provides patient care in a clinic setting. Functions within the scope of training received from Medical Assistant education program and from Gonzaba Medical Group training programs including, but not limited to obtaining vital signs, applying monitoring devices such as pulse oximeter and EKG leads/pads, glucose monitoring, retinal exams, and QuantaFlo testing.
  • Reports abnormal values and accurately documents clinical findings to provider in charge.
  • Assists in providing quality health care by accessing the electronic medical record to obtain/provide pertinent information.
  • Remains aligned with the departmental goals which involves understanding the importance of conducting RMH visits, perform HEDIS measures, and participating providing quality care by utilizing the tools and reports provided to help reach

departmental/organizational goals.

  • Maintains assigned work area with supplies and equipment. Cleans, stocks, and maintains

exam rooms, procedure rooms, instruments, and equipment.

  • Follows universal precaution standards about patients, procedures, handling specimens, cleaning exam rooms as taught and retrained through annual compliance training.
  • Transports patients safely from the following positions: sitting to standing or laying down, from standing to sitting or laying down, from laying down to standing or sitting, and assist to the bathroom, into/out of wheelchairs, and pushing/pulling patients in wheelchairs.
  • Must always keep the patients’ safety a priority, which will involve preventing potential falls as needed.
  • Will need to travel to support patient care at all GMG locations on a rotational basis.
  • Maintains strict confidentiality.

Work Environment: Clinic environment. Exposure to communicable diseases, bodily fluids, toxic substances, ionizing radiation, medicinal preparations, and other conditions common to a clinic environment.

Mental / Physical Requirements: Requires manual dexterity, sitting, standing, stooping, reaching, kneeling, crouching, bending, walking, lifting up to 40 lbs. without assistance. Close vision and ability to adjust focus. Must be able to work efficiently under pressure.
